Below is a list of the required orientation forms and resources. At your convenience, please download and complete each form including the appropriate tax forms. Please submit all completed payroll forms to humanresources@uscourts.cavc.gov before the start of orientation. Please print and bring benefit forms to orientation - further instruction will be given during orientation.

How to Claim Prior Federal Service
In order to verify prior federal service listed on your SF-144, we will need copies of your last couple of SF-50s. If you are coming to the Court without a Break in Service (a separation equal to or less than 3 calendar days between prior agency and new agency), please provide:

How to Claim Veterans' Preference
When claiming Veterans' Preference, you must provide a copy of your DD-214 (Member 4 Copy), certificate of honorable release or discharge from active duty or other acceptable documentation. Applicants claiming 10-point preference will need to submit Form SF-15 or other acceptable documentation. The DD-214 is issued to military members upon separation from active service.
In addition, if you are a widow/widower of a veteran, the spouse of a disabled veteran or the natural mother of a disabled or deceased veteran, you must submit a Standard Form (SF) 15, "Application for 10-Point Veteran Preference," and the other required documentation identified on the reverse side of the SF-15 to support your preference claim.
Failure to provide the supporting documents will result in Veterans' Preference not applied to your profile
