The Notice of Appeal form and Declaration of Financial Hardship have been updated.
Due to public health concerns associated with the spread of the COVID-19 virus, the Court would like to fully utilize electronic forms of communication. More detailed instructions have been added to the forms so pro se appellants and petitioners may take advantage of the fax and email options for submissions to the Court. Pro se appellants and petitioners may fax or email the forms and other pleadings to the Court to 202-501-5848 and
self-rep@uscourts.cavc.gov. Appellants and petitioners with email addresses may also receive email notifications of docket activity from the Court.
